I certify the billing/mailing address is accurate and has also been updated at the US Postal service before I submitted this request. I understand that if there is not a new application for service the water could be off as early as 8:00 AM on the Date Requested. I further certify that the above account is for only one premise (house, store, apartment etc.). If OWASA's water meter serves more than one premise, I hereby certify that I am the owner or property manager, and the property is entirely vacant. I further acknowledge that I understand that once the water is off for such a property, only the owner can re-establish service.
